At Coyote Counseling, we understand that clients may need documentation for various personal, medical, academic, or employment-related matters.

When it’s clinically appropriate, we’re here to support those requests with the same presence, integrity, and thoughtful attention we bring to our therapeutic work.

What These Services Include

Administrative documentation services are available for needs such as:

  • FMLA paperwork

  • Workplace or school accommodations

  • Short-term disability forms

  • Letters for emotional support or functional impact (when clinically appropriate)

  • Record summaries or treatment verification

  • Other documentation requiring clinical judgment or formal assessment

These services exist outside of regular therapy sessions because they require dedicated time, detailed review, and coordination to ensure that what we provide is accurate, ethical, and truly supportive of your care.

Not every request will be clinically appropriate, and when that’s the case, we’ll talk through it with transparency and compassion. Our goal is to support you, not to simply sign forms, but to help you navigate these systems in a way that aligns with your wellbeing and your healing.

  • 15‑30 minutes: $40-75

Moderate paperwork (e.g., brief forms, short accommodations requests):

  • 30‑60 minutes: $80-150

More extensive documentation or evaluation (FMLA evaluation, short‑term disability, detailed report):

  • 1‑2 hours or more: $150-300
